Admissions Physical Therapist

Conduct comprehensive OASIS Start of Care admissions visits for adult and geriatric home health patients across Essex County, NJ. Perform detailed physical therapy evaluations covering mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities. Develop individualized treatment plans based on patient needs and physician orders, then collaborate with nursing and interdisciplinary teams to coordinate care. Educate patients, families, and caregivers to promote independence and safety while accurately documenting clinical findings in electronic medical records. Continuously evaluate patient progress and adjust care plans as appropriate.
