Job Summary
Facilitate the admissions process for new residents in a skilled nursing facility: conduct comprehensive assessments, collaborate with physicians and nursing staff to develop individualized care plans, complete and verify admission documentation, coordinate with dietary and therapy for required services, administer medications as prescribed, monitor vital signs and overall condition, provide emotional support to residents and families, ensure regulatory compliance, and participate in team discussions on discharge planning and resident progress.
Required Qualifications
- Valid Registered Nurse (RN) license in the state of work
- Strong assessment and critical thinking skills
- Knowledge of regulatory requirements governing admissions in skilled nursing facilities
- Ability to build rapport with patients and families
- Excellent interpersonal and communication skills
- Proficiency with electronic medical record systems
