Job Summary
Conduct hands-on patient assessments and initiate care plans during admissions and readmissions while accurately receiving and transcribing physician orders. Deliver quality nursing care, communicate findings to physicians, and chart findings timely to ensure medication implementation. Coordinate patient care through interpersonal contact with patients, families, and staff, and assist in patient evacuations as needed. Maintain confidentiality, adhere to Life Care standards, and function productively on an interdisciplinary team. Requires an active RN license, one year of geriatric nursing experience preferred, and CPR certification. Weekend schedule with on-call rotation.
Required Qualifications
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State
- Must maintain an active Registered Nurse (RN) license in good standing throughout employment
- Expert knowledge in field of practice
- Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
- Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
- Maintains professional working relationships with all associates, vendors, etc.
- Maintains confidentiality of all proprietary and/or confidential information
- Understand and follow company policies including harassment and compliance procedures
- Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training
- Provide hands on assessment care to patients during the admissions process
- Effectively communicate assessment findings with physician
- Knowledgeably and competently deliver quality nursing care to patients
- Evaluate patients' needs assessment and initiate care plans upon patient admission and readmission
- Accurately receive, document, and transcribe physician orders
- Communicate orders to others timely to ensure timely receipt of medications and order implementation
- Chart appropriately and timely
- Exhibit excellent customer service and a positive attitude towards patients
- Assist in the evacuation of patients
- Demonstrate dependable, regular attendance
- Concentrate and use reasoning skills and good judgment
- Communicate and function productively on an interdisciplinary team
- Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
- Read, write, speak, and understand the English language
Desired Qualifications
- One (1) year geriatric nursing experience preferred
