Job Summary
Admissions Nurse at Montello Care Center coordinates admissions, tours, and transitions from intake to discharge, acts as liaison for patients and families, manages room readiness, coordinates equipment and DME, completes PASRR and census events in MatrixCare, handles admission agreements and consents, performs initial admission assessments and medication reconciliation, and conducts follow-up to ensure satisfaction and successful discharge; requires active LPN or RN license in Wisconsin, CPR certification, and at least 2 years of healthcare experience; position includes on-call weekend rotation.
Required Qualifications
- Active LPN or RN licensed in the State of Wisconsin
- Maintained current CPR certification
- Minimum 2 years of experience in healthcare, post-acute or long-term care
- Ability to relate positively with residents, patients, families, community members, volunteers and staff
- Able to read, write and speak English
- Basic computer knowledge and ability to learn company applications
