Admissions Nurse (Hospice)

Job Summary
Admissions Nurse at Lifted Hospice serves as the first clinical contact for patients and families entering hospice. You guide patients through the transition into hospice with clarity, compassion, and confidence, perform timely nursing assessments, develop and document a personalized plan of care with the patient, family, and interdisciplinary team, deliver skilled care with proper infection control in line with OSHA standards, collaborate with medical directors and hospice leadership, ensure accurate documentation and compliance with regulatory standards, support families through education and clear communication, participate in on-call rotations, and pursue ongoing training and quality improvement to maintain clinical excellence. The role emphasizes local travel to patient homes or facilities and requires the ability to adapt to diverse settings while delivering compassionate, patient- and family-centered admissions care.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program
- active state nursing license
- at least two years of RN experience
- hospice experience preferred
- strong assessment, critical thinking, and communication skills
- familiarity with hospice regulations (CoPs)
- ability to work independently and manage deadlines
- travel locally to complete admissions
- read, write, and communicate in English
