Admissions Nurse – Help Make Every Transition Seamless

Job Summary
Review clinical information and referral documentation for prospective admissions to determine if the center can meet patient needs. Communicate with hospitals, physicians, case managers, social workers, referral sources, patients, and families regarding potential admissions. Collaborate with nursing leadership to coordinate safe and timely admissions, ensuring required documentation is complete and accurate. Coordinate clinical information with the receiving nursing team prior to patient arrival and follow up on missing documentation. Identify special equipment, medication, or treatment needs before admission and maintain accurate records of referrals and decisions. Provide professional, compassionate communication throughout the process while adhering to NHC policies and regulatory requirements. Support a smooth transition into the center with additional duties as assigned.
Required Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Tennessee, or a current multistate/compact license that permits practice in Tennessee.
- Graduate of an approved nursing program.
- Current CPR certification as required.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and interpersonal skills.
- Strong organization and attention to detail.
- Ability to review clinical documentation and manage multiple referrals and priorities.
- Ability to work collaboratively with patients, families, referral sources, and interdisciplinary healthcare teams.
- Professional, compassionate, and team-oriented approach.
Desired Qualifications
- Previous long-term care, skilled nursing, rehabilitation, hospital, admissions, or case-management experience preferred.
