Admissions & Medical Records Nurse (LPN, RN)

Assess new residents and review baseline care plans with nursing leadership. Implement preventative measures for high-risk residents using Braden Scale results and ensure fall risk assessments trigger required interventions. Complete nursing admission assessments, MDS section GG functional assessments, and resident documentation in MatrixCare. Verify census events in the Electronic Health Record and update CNA sheets/profiles. Provide patient care as needed and participate in campus on-call rotation. Make written and oral reports to nursing leadership regarding department operations.
