Job Summary
Admissions Intake Representative role for Hospice Savannah requiring an LPN license; manages intake processes for admissions, reviews all clinical records for eligibility, reviews payer sources, obtains pre-certifications, handles incoming calls, schedules visits, and educates patients and families on services within Hospice Savannah. Collaborates effectively with an interdisciplinary team and ensures compliance with Joint Commission standards and regulatory requirements. Demonstrates strong communication, patient education, and problem-solving skills with a focus on end-of-life care.
Required Qualifications
- Licensed Practical Nurse (LPN) license required
- Graduate of an accredited school of Nursing
- CPR certification required
