Admissions/Discharge Coordinator (RN) Shift: 11:00 AM – 11:00 PM Rotating Shifts

Job Summary
Coordinate admissions, transfers, and discharges for residents; provide clinical and operational leadership during the 11:00 AM – 11:00 PM shift; supervise nursing staff in the absence of the DON; ensure accurate and timely clinical documentation; review staffing at shift change and adjust assignments; conduct patient care rounds and resolve clinical/operational concerns; monitor staff performance and initiate coaching or corrective action when necessary; communicate shift updates to the DON, ADON, and Unit Managers; participate in committee meetings and support overall facility operations.
Required Qualifications
- Active nursing license (RN preferred, not required)
- Strong clinical assessment and leadership skills
- Experience in long-term care or skilled nursing preferred
