Job Summary
Admissions Coordinator role focused on processing incoming SNF referrals, verifying insurance eligibility, obtaining prior authorizations, and entering referral data into the EMR, with ongoing coordination with the Admissions Team and Patient Transition Liaisons. Requires interaction with residents, families, staff, and external partners while maintaining confidentiality, monitoring records and compliance, supporting audits and quality meetings, identifying improvement opportunities, and contributing to policy development. Demonstrated proficiency with health insurance plans and healthcare experience (skilled nursing or assisted living) is preferred; strong computer skills, time management, and a customer-service mindset are essential.
Required Qualifications
- High school diploma or equivalent
- Proficient computer skills and excellent time management
- Experience with health insurance, Medicare, Medical Assistance, and other health care plans is preferred
- Experience in healthcare, skilled nursing facility, or assisted living community
- Experience processing referrals, verifying insurance eligibility, conducting background checks, obtaining prior authorizations, and entering referral information into an electronic medical record (EMR) is required
- Strong communication with residents, families, staff, and external partners; maintain confidentiality; adhere to compliance standards
- Support departmental records, audits, inspections, and quality improvement initiatives
- Participation in departmental and quality assurance meetings; contribute to policy development
- Customer service oriented with a positive, inclusive workplace culture
