Admissions Coordinator

Plan, develop, organize, implement, and evaluate activity programs while observing resident attendance, mood, and behavior to chart progress. Interview residents or family members to gather activity information and involve them in planning objectives and goals. Participate in discharge planning, activity care plan development, and community planning related to facility services. Assist in arranging transportation to other facilities as needed. Requires one year of experience in a Skilled Nursing Facility and one year in an Admissions role.
