Job Summary
Coordinate admissions, intake, and transition processes for new individuals entering services; assist families and stakeholders with benefits and entitlement processes, including Medicaid and related documentation; monitor eligibility requirements, track timelines, and maintain organized records to support continuity of services; serve as a key point of contact for families, care coordinators, hospitals, and referral sources; collaborate with interdisciplinary teams to ensure smooth transitions, accurate documentation, and timely service coordination.
Required Qualifications
- Experience in healthcare, human services, admissions, benefits coordination, case management, or a related field preferred
- Knowledge of Medicaid, Social Security benefits, entitlement systems, or OPWDD-related services strongly preferred
- Strong communication, organizational, and follow-through skills
- Ability to manage multiple priorities and maintain confidentiality
- Associate’s or Bachelor’s degree preferred depending on experience
