Job Summary
Admissions Coordinator for a homelessness recuperative care facility within Pacific Pointe Senior Apartments. Responsibilities include serving as the primary contact for admissions, conducting initial screenings to determine eligibility, coordinating with healthcare and social service providers to facilitate referrals, collecting and reviewing documentation (medical records, insurance), communicating with residents and support networks, collaborating with the interdisciplinary team to develop care plans, and maintaining accurate admission records in compliance with regulatory requirements.
