Admissions and Outreach Manager

Job Summary

Review prospective clients for outpatient admission, coordinate intakes and medical appointments, and conduct substance use and mental health assessments using standardized tools. Supervise Peer Specialists, complete evaluations, and perform chart audits to ensure accurate documentation and quality assurance. Collaborate with the treatment team to develop comprehensive service plans, while explaining program rules, policies, and treatment service agreements to clients. Complete phone screenings, comprehensive psycho-social assessments, and monthly orientation groups for new admissions. Monitor census reconciliation between systems, process client ID cards, and conduct random toxicology testing as necessary. Participate in case reviews, interdisciplinary team meetings, and ongoing professional development to support client compliance and progress.

Required Qualifications

  • QHP required: LMSW, MSW eligible for licensing, unrestricted OASAS CASAC or another equivalent QHP license
  • A minimum of 3 years work experience in the addiction or counseling field
  • one of which shall be in a supervisory role
  • Must obtain Mandated reporter (2 hours) training/certificate prior to hire date
  • Must obtain training/certification within thirty (30) days of hire
  • Domestic Violence
  • Infection Control BASIC (non-medical/nursing staff)
  • HIV
  • Child Abuse
  • Tobacco Cessation
  • Impaired Driver Screening and Assessment
  • Must obtain training/certification during in-house orientation
  • Supporting Recovery with Medications Addictions Treatment (MAT)
  • Must obtain SUD training/certificate within thirty (30) days of hire (if applicable)
  • Screening, Brief Intervention and Referral to Treatment
  • CASAC Canon of Ethics (6 hours)
  • Confidentiality related to 42CFR (3 Hours)
  • Excellent organizational, communication and time management skills
  • Ability to work on a resolute team of professionals in a culturally diverse environment
  • Ability to multitask and to work and function under pressure
  • Working knowledge of electronic health record Nextgen

Desired Qualifications

  • Bilingual - Spanish preferred
