Job Summary
Admissions RN role responsible for establishing relationships with community referral sources, promoting hospice access, coordinating admissions, and providing professional nursing care for hospice patients. Responsibilities include assessing patient/family needs, coordinating with ED/DPCS and IDT to plan care, facilitating hospice referrals and admissions, educating patients and families about hospice services, and ensuring timely, high-quality care coordination across facilities. Requires current California RN license, BSN or equivalent nursing experience, ability to travel up to 20%, and flexibility in work hours. Schedule is Friday–Sunday 10:00am–9:00pm in Fresno, CA.
Required Qualifications
- Current RN license in the state of California
- Bachelor’s degree in Nursing from NLN-accredited program OR minimum of one (1) year professional nursing experience within the last three (3) years
- Willingness to travel up to 20% of the time
- Flexibility in work hours
- Ability to read and follow written instructions
- Strong interpersonal and communication skills
- Knowledge of hospice philosophy and provision of specialized care to terminally ill patients
