Job Summary

Complete clinical admissions for all Company services, including explaining services to potential patients, identifying entry barriers, and obtaining informed consents. Initiate skilled nursing interventions to prevent complications and maximize patient comfort while obtaining physician orders and completing documentation per policy. Act as the Company representative at assigned facilities, facilitating referrals to all service lines and communicating frequently with Care Navigation staff to ensure timely follow-up. Provide care for GIP patients in hospitals, including daily visits, professional management, and efficient discharge, while obtaining medical record information for referrals.

Required Qualifications

  • Current license as RN in the state where the employee will be working
  • Minimum of one (1) year nursing experience
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Strong clinical assessment skills
  • Ability to effectively communicate in English (verbal and written)
  • Valid driver's license
  • Automobile insurance per Company policy
  • Reliable transportation to meet schedule
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Desired Qualifications

  • Hospice or hospital experience
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Background in emergency department or ICU
  • Bi-lingual in Spanish
  • Ability to present services in a professional and articulate manner
  • Strong goal/outcome orientation with exceptional time management skills
  • Excellent customer service focus
