Job Summary
Conduct comprehensive hospice admission assessments including physical, psychosocial, and medical history evaluations. Develop individualized care plans and initiate skilled nursing care, symptom management, and patient education for home-based patients. Collaborate with physicians and interdisciplinary teams to coordinate services while monitoring and documenting patient status and interventions. Communicate condition changes to obtain physician orders as needed. Requires current California RN license, 2+ years of RN experience, and CPR/BLS certification. Full-time role with $50-$65 per hour pay, 10:00 am to 6:30 pm schedule, and on-call rotation.
Required Qualifications
- Current and unrestricted California Registered Nurse (RN) License
- Minimum of 2 years of RN experience
- Strong clinical assessment, care planning, and communication skills
- Current CPR/BLS Certification
- Valid driver's license
- Reliable transportation
- Current auto insurance
- Ability to travel to patient homes
- Ability to perform physical job duties, including standing, bending, lifting, and transferring patients as needed
