Job Summary
Admission Nurse coordinates hospice admissions by meeting with patients and families to explain services, conducts comprehensive admission assessments, arranges initial nursing visit frequency, DME/RX needs, and assesses bereavement, social work, chaplain, and volunteer services. Responsibilities include educating patients/families on hospice services, reporting to the Interdisciplinary Team, completing thorough documentation, and possibly conducting nursing or 'tuck-in' visits. May train admission staff, supervise LVN/CNA staff, and perform supervisory visits as part of the role.
Required Qualifications
- Must be a Registered Nurse in the state of Texas
- Hold a valid driver’s license
- 1 year of nursing experience required
- Case Management experience required
- Strongly prefer 2+ years of experience in hospice case management
- Must have proficient computer skills and Microsoft Office suite experience
