Job Summary
Adherence Navigator (Social Worker) provides remote, telephone- and message-based patient support following approved talking points to educate on condition, treatment, and adherence barriers. Utilizes motivational interviewing and cognitive behavioral therapy techniques to promote positive patient outcomes, documents interactions in a CRM, protects PHI, and adheres to HIPAA and program protocols. Key duties include building trust with patients and caregivers, addressing questions about injection concerns, side effects, cost, and HCP communication, and ensuring high-quality service through monitoring and compliance. Requires a Bachelor of Social Work, experience counseling patients by phone/video, CRM proficiency, cardiovascular-patient counseling experience, and (preferred) Spanish language skills; remote role with EST hours and potential travel as needed.
Required Qualifications
- Bachelor of Social Work (minimum)
- Previous experience counseling patients over phone and/or video
- Proficient in use of CRM technology
- Experience counseling cardiovascular patients
- Experience with patient adherence counseling
- Bilingual (Spanish) preferred
- 2 years professional experience
Additional Requirements
- Some travel may be required
- Work from home position, dedicated office space required
- Must be able to work 11 am to 8 pm EST
