Job Summary
Transitional Aged Youth Case Manager (Addictions Counsellor) — full-time, 37.5 hours/week, hybrid in Oakville, Ontario, with travel across Halton Region. Delivers clinical assessments and develops and implements treatment plans for Transitional Aged Youth ages 16–24, providing intensive case management in an outreach service model. Works with the ADAPT TAY Coordinator/Youth Addictions Manager, project partners, and a broad network of service providers to increase service integration. Responsibilities include specialized assessments, developing individual Treatment Plans, individual/couples/family/group counselling (DBT skills training is an asset), coordinating referrals, participating in multi-agency TAY consultations, maintaining records, promoting ADAPT programs, contributing to operational planning, and supporting community education and public awareness initiatives. Requires university degree/diploma in Social Work or health-related field with addictions/c oncurrent disorders training, relevant certifications (CPR/First Aid, ASIST, CPI asset), and minimum 2 years’ experience with youth in addictions or related fields. Strong organizational, documentation, cultural responsiveness, and communication skills; ability to work flexible hours, including evenings and some weekends, from a hybrid work setting.
Required Qualifications
- Education: University degree/ diploma in Social Work and/ or Health-related field plus Addiction/ Concurrent Disorders studies certification, or equivalent combination of experience and education.
- Licenses, certification, credentials: Registered Social Service Worker is an asset; Current CPR and First Aid certification is required; ASIST training or equivalent is required; CPI training is an asset; CACC certification is an asset
- Experience: At least 2 years working experience with youth and young adults in the area of addictions or related field; Experience in: Community development and networking; Record keeping and file management; Basic life skills counselling, supportive counselling, crisis intervention, advocacy, and group work; Applied knowledge of addictions treatments and explanatory theories, mental illness, and community resources; Managing clients with complex needs in varying stages of change
- YOUR SKILLS AND KNOWLEDGE: Basic life skills counselling, supportive counselling, crisis intervention, advocacy, and group work; Managing clients with complex needs in varying stages of change; Ability to effectively present complex clinical information in writing and verbally; Ability to read and understand documentation such as operating instructions, procedures manuals and complex clinical reports; Broad knowledge of addictions treatments and explanatory theories, mental illness, and community resources; Ability to function independently, maintain effective working relationships with others, work effectively under pressure or deadlines and work outside regular office hours as required; Interpersonal skills with attention to detail and rapport building; Ethical boundaries and clinical reporting obligations; Culturally responsive services; Knowledge of the history of discrimination in America and related disparities; Knowledge of French language is an asset; Proficiency with computer programs and electronic databases; Police record check; Strong organization and time management; Attention to detail; Ability to build rapport and active listening; Remote/work from home accommodation flexibility; Stakeholder engagement and collaboration; Ability to work evenings/weekends as required
