Job Summary
Conduct community-based substance use evaluations, therapy, and medication administration within the PACT Forensic model. Provide comprehensive risk/crisis assessments, facilitate dual recovery groups, and assist with Activities of Daily Living in client homes, hospitals, and community settings. Coordinate care with family members, residential providers, and community agencies while monitoring progress toward treatment plan goals. Act as a liaison to ensure positive outcomes and safety for individuals served, including those with forensic involvement. Maintain professional licensure and complete required clinical education hours. Work Monday through Friday with no weekends required.
Required Qualifications
- Bachelor's Degree
- CADCH
- Valid driver's license
- reliable vehicle available for all working hours

- Minimum of six months' experience in human services
- Commitment to empowerment and growth, human rights, and community integration of individuals served
- Demonstrates willingness and ability to treat challenging and diverse client populations, including a forensic population
- Demonstrates cultural competence in supporting clients with a variety of different cultural backgrounds
- Demonstrates working knowledge of community resources, and how to help clients access them
- Demonstrates willingness to work with individuals who are forensically involved within the Sequential Intercept model
- Excellent verbal and written communication skills
