Job Summary
Conduct comprehensive health assessments, order and interpret diagnostic tests, and manage treatment plans for cardiology patients in an outpatient setting. Provide patient education on disease prevention and lifestyle modifications, coordinate care with specialists, and document interactions in electronic health records. Monitor chronic conditions, follow up post-procedures, and adjust treatment plans as necessary while participating in quality improvement initiatives. Licensed in Illinois with ANCC/AANP or NCCPA certification required. Hourly range $59.00-$62.50.
