Job Summary
Evaluate, diagnose, and manage patients with acute and chronic kidney disease while collaborating closely with nephrologists and interdisciplinary teams. Develop and implement treatment plans, provide patient education on disease management, and document encounters accurately within the EMR. Participate in care coordination across outpatient and inpatient settings and manage a subset of dialysis patients. Work Monday through Friday in a daytime schedule with primarily office and inpatient-based care, with no routine weekend coverage. Bilingual Spanish-speaking candidates are strongly encouraged to apply.
Required Qualifications
- Master's or Doctoral degree in Nursing from an accredited program
- Current Acute Care Nurse Practitioner certification (AGACNP)
- Active Texas Nurse Practitioner license or eligibility to obtain
- Minimum of one year of Nurse Practitioner experience
- Strong clinical assessment, diagnostic, and care management skills
- Excellent communication and collaboration abilities
- Commitment to delivering high-quality, patient-centered care
Desired Qualifications
- Nephrology or dialysis experience preferred, but not required
- Bilingual Spanish/English preferred
