Job Summary
Manage patient outcomes and coordinate care across the continuum for seniors in acute and post-acute settings. Daily presence at assigned hospitals includes identifying inpatient versus observation status, assessing functional needs, and facilitating safe discharge to Skilled Nursing Facilities, Long-Term Care, or home. Collaborate with hospitalists, specialists, and community providers to plan next-level care, perform Social Determinants of Health screenings, and coordinate physician follow-up appointments. Document daily updates in the EMR, conduct bedside discussions on disease management programs, and execute the ACM Coaching initiative. Travel locally, regionally, and nationwide up to 10% of the time to support care transitions and ensure seamless handoffs to Community Care Nurses.
Required Qualifications
- Bachelor's Degree in nursing (BSN) or RN with bachelor's degree in a related clinical field
- A valid, active Registered Nurse (RN) license in State of employment
- A minimum of two (2) years' clinical work experience
- A minimum of one (1) year of utilization review and/or case management, home health, hospital discharge planning experience
- A minimum of one (1) year of case management experience in acute case management or community case management
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
- Current, valid driver's license
- Spoken and written fluency in English
- Proficient in Microsoft Office Suite products including Excel, Word, PowerPoint, and Outlook
- Ability and willingness to travel locally, regionally, and nationwide up to 10% of the time
Desired Qualifications
- Bachelor's degree in a related clinical field preferred
- Compact License preferred for states where compact license is available
- Certified Case Manager certification
- Certification through the Commission for Case Manager Certification (CCMC) or the American Association of Managed Care Nurses (CMCN)
- Bilingual preferred
