Job Summary
Coordinate services for individuals with mental health and substance use disorders; provide service coordination for individuals, develop and implement the overall service plan, assist in developing natural support networks, individual and group counseling, monitoring symptoms and medications, linking individuals to services, writing and updating Person Centered Plans, functioning as part of a multi-disciplinary team, facilitating independent living skills, coordinating with programs and facilities, ensuring documentation, and attending meetings; travel may be required; Bachelor’s in Human Services required with relevant experience; CADC/LCAS/LP qualifications preferred.
Required Qualifications
- Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience

- Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience
- Certified Alcohol & Drug Counselor (CADC) - State Addictions Specialist Professional Practice Board
- Licensed Clinical Addiction Specialist (LCAS) - State Substance Abuse Professional Practice Board
- Licensed Clinical Addiction Specialist - Associate (LCAS-A) - State Substance Abuse Professional Practice Board
- Qualified Professional (QP) - Monarch-DSM
