Job Summary
Deliver nursing services, case management, and supportive counseling to individuals served by the Assertive Community Treatment Team. Administer and monitor medications, review laboratory values, and evaluate physical health while making referrals to community physicians. Conduct daily intake assessments covering medical, psychiatric, and vocational histories; ensure immediate interventions for changes in mental status. Coordinate services with inpatient facilities, detox programs, and community resources to establish support networks. Act as an after-hours liaison, meet regularly with team members to formulate plans, and document participant activities and outcomes per service records. Provide supervision to LPN staff as needed.
Required Qualifications
- Associates degree in Nursing
- Valid Driver's License (USA)
- Registered Nurse (RN) certification
- Experience working with adults with mental illness (1 Year)
- Psychiatric Nursing Experience (1 Year)
- Working knowledge of psychiatric medications
- Driving and travel
