Job Summary
Activity Leader for On Lok PACE who plans, develops, implements, and oversees activity services for participants. Responsibilities include evaluating and implementing individualized participant care plans, training activity aides, planning and offering varied monthly programs, conducting group and individual interventions to maximize cognitive and functional abilities, coordinating monthly community outings and special events with Activity Therapist and Volunteer Coordinator, documenting participant involvement, revising care plans, and participating in team discussions and department meetings. Requirements include multi-year experience in social/recreational/educational programs with health-care exposure, or relevant training/credentials, ability to work with volunteers and contract staff, and English fluency with bilingual preference. Required Qualifications
- Minimum of two years' experience in a social, recreational or educational program within the past five years, one year full-time in a health care setting; OR completion of at least 36 hours of training in an activity leadership course from an accredited college; OR qualifications as an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, occupational therapy assistant or a qualified social worker
- One-year experience working with the elderly or frail population
- Written and oral fluency in English; bilingual (Cantonese or Spanish) preferred
