Job Summary
Deliver programs and activities supporting habilitation, wellness, and recreation across Adult Day Health sites, participant homes, and via telehealth; complete comprehensive assessments and individualized treatment plans; plan and implement daily activities, coordinate delivery with staff, and participate in IDT meetings; develop and write participant assessments, progress notes, care plans, and reports; plan special events and maintain documentation and tracking; require OTA credential and MA licensure with CPR and geriatric program-development experience.
Required Qualifications
- O. T. A. curriculum approved by the Accreditation Council for Occupational Therapy Education, Inc. with a minimum of an Associate’s Degree.
- Maintains current Occupational Therapy Assistant licensure in the Commonwealth of MA
- Maintains current CPR Certification
- Minimum of 1-2 years of experience working with a geriatric population.
- Minimum of 1-2 years of experience in program development.
