Job Summary

Assist clients with mobility, transportation, and activities of daily living during programming while conducting continuous informal safety assessments and responding to emerging concerns using de-escalation procedures. Plan, implement, and evaluate recreational, social, intellectual, emotional, and spiritual programs by actively seeking client input to ensure flexibility and responsiveness to unique needs and preferences. Facilitate community-based activities that encourage social inclusion and adapt interactions to align with each client's cultural background, belief systems, and communication styles. Maintain accurate documentation including daily communication logs and participation notes, and clean, organize, and maintain the activity space and equipment.

Required Qualifications

  • High school diploma or GED
  • Valid certification as Certified Nursing Assistant (CNA)
  • Valid certification as Nursing Assistant Certified (NAC)
  • Valid certification as Home Care Assistant (HCA)
  • Two years' experience working with vulnerable populations
  • Food Handlers Card
  • First Aid and CPR certification
  • Proof of negative Tuberculosis (TB) test within the past 12 months
  • Ability to be compassionate, nonjudgmental, and set clear personal boundaries
  • Ability to set clear limits and consequences and manage triangulation with guests and staff
  • Ability to take strong initiative and ownership of the position, be proactive in problem solving, and be an effective team communicator
  • Ability to form and maintain a strong team commitment with staff
  • Ability to work with diverse populations, including people with active mental health and chemical dependency issues
  • Ability to be a positive and proactive team member
  • Ability to support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services
  • Ability to support and contribute to a creative, collaborative, and respectful environment that promotes teamwork
  • Ability to demonstrate the necessary attitudes, knowledge, and skills to deliver culturally competent services and work effectively in multi-cultural situations

Desired Qualifications

  • Associates Degree in Social Service or similar field
  • Experience working in trauma-informed care
  • Experience working in wraparound services
  • Experience working in person-centered planning
