Job Summary
Conduct therapy, provide crisis counseling, and deliver medication support to clients with chronic mental illness via home visits and community outreach. Assist clients in securing affordable housing, competitive employment, and medical/dental appointments while teaching self-advocacy, coping skills, and relapse prevention strategies. Develop individualized service plans, lead family psycho-education groups, and coordinate multi-disciplinary team goals to promote recovery and community integration. Maintain accurate documentation, manage client charts, and process correspondence as required. Requires 24/7 coverage including weekends and holidays, a valid driver's license, and a minimum of six months of direct care experience.
Required Qualifications
- Minimum 6 months of direct care experience with serious mental health illness
- Valid Driver's license
- Clean driving record
- Reliable vehicle
- Ability to drive frequently
- Ability to sit and type at a computer frequently
- Ability to sit, stand, and walk frequently
- Ability to climb flights of stairs
- Ability to lift a minimum of 40 pounds
- Proper body mechanics for personal and patient/client safety
- Ability to comply with DOH requirements for annual health assessments
- Ability to comply with required immunizations
- Ability to attend all mandatory classes, meetings and committees
- Ability to complete department-specific requirements/competencies
