Job Summary
The ACT Substance Use Disorder Specialist provides integrated substance use treatment services to individuals with co-occurring mental health and substance use disorders within a multidisciplinary, community-based team. Responsibilities include substance use screening, assessment, and ongoing treatment; delivering evidence-based interventions (Motivational Interviewing, Harm Reduction, relapse prevention); supporting development and implementation of integrated treatment plans; providing individual and group SUD services in community settings; coordinating care with detox, residential, outpatient, and recovery support services; supporting MAT coordination as appropriate; psychoeducation related to substance use and recovery; crisis intervention and stabilization; participating in daily ACT team meetings and shared caseload responsibilities; delivering services in the community in alignment with ACT fidelity standards; collaborating with internal and external partners; maintaining timely documentation in accordance with Medicaid and agency requirements; and travel within the designated service area. Services are delivered primarily in the community (minimum 75%). The role emphasizes high-frequency, individualized service delivery based on acuity, assertive outreach and engagement strategies, and focus on reducing hospitalizations and readmissions. Beneficial qualifications include excellent communication and organizational skills, Medicaid billing ability, and a valid driver’s license with appropriate auto insurance. Required Qualifications
- High school degree or GED equivalent is required
- Bachelor�s degree in behavioral health, healthcare or related fields is preferred
- Chemical Dependency Licensed in the state of Ohio as a CDCA, LCDC II, LCDC III is required
