Job Summary
ACT Team Registered Nurse providing community-based nursing services for adults with serious mental illness. Serve as a vital connection between psychiatric and physical healthcare by administering medications (including long-acting injectable antipsychotics), monitoring health and treatment plans, coordinating with primary care and specialty providers, conducting nursing assessments and health screenings, and delivering education on medications, chronic disease management, nutrition, wellness, and preventive care. Collaborate with psychiatrists, therapists, case managers, peer support specialists, and substance use counselors; participate in daily interdisciplinary team meetings and on-call rotations; document accurately in the Electronic Medical Record; travel within the community to client homes, shelters, and other locations. The role emphasizes recovery-oriented, trauma-informed care within a highly collaborative, community-based setting in Salt Lake City, Utah.
Required Qualifications
- Current, unrestricted Registered Nurse (RN) license in the State of Utah
- Bachelor's Degree in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience may be considered
- CPR/BLS Certification
- Valid Driver's License and insurability
- Ability to travel throughout the community to provide services
Desired Qualifications
- Two or more years of nursing experience
- Experience in psychiatric nursing, community mental health, behavioral health, inpatient psychiatry, or crisis services
- Experience administering long-acting injectable (LAI) medications
- Experience working with individuals experiencing co-occurring mental health and substance use disorders
- Familiarity with Medication-Assisted Treatment (MAT)
- Psychiatric Mental Health Nursing Certification (RN-BC)
