Job Summary
ACT Rehabilitation Specialist on an Assertive Community Treatment (ACT) team responsible for helping members obtain and maintain involvement in employment and/or community-based activities. Provides on-call crisis intervention services with weekend/holiday coverage. Collaborates with team members, including Prescribing Clinician and Nurse, to develop and implement individual service plans and crisis plans. Coordinates with families and community agencies, facilitates rehabilitation skills training, and assists members in accessing services and entitlements. Also responsible for transporting members to necessary services as outlined in their service plans.
Required Qualifications
- HS Diploma/GED and six (6) years of full-time direct care experience.
- Bachelor’s Degree in a related field OR a bachelor’s degree in a non-related field with two (2) years experience preferred.
- Must have one (1) year of training or experience in vocational rehabilitation and support.
- Prior fingerprint clearance or eligibility to receive one within eight weeks of hire.
- Reliable transportation and valid AZ Driver’s License with state minimum insurance coverage.
- TB Test clearance at time of hire.
