Job Summary
Provide concrete service delivery to clients (benefits/entitlements) and lead/co-lead recovery groups on issues like self-advocacy, housing, employment readiness, and substance use; document notes in electronic health records; travel 80% in the community and conduct visits in shelters, clients' homes, or other community locations; train consumers in peer advocacy and harm reduction; escort clients to doctors, courts, and appointments; teach independent living skills (cooking, laundry, grocery shopping, medication adherence); conduct relapse prevention planning and search efforts for missing clients; participate in team meetings and discharge planning; perform general program duties and maintain recovery resources. Requires NY Peer Specialist certification and relevant experience in community-based services.
Required Qualifications
- High School Diploma or GED or some post-secondary education preferred
- NY Peer Specialist certification is required
- experience with a similar population in a community setting
