Job Summary
Provide in-home nursing services to clients with severe and persistent mental illness by administering medications, assessing side effects, and delivering health education. Conduct basic medical assessments including vital signs and venipuncture under prescriber direction while managing medication storage and pharmacy coordination. Deliver therapeutic interventions in crisis situations, assist with hospital diversion and discharge planning, and maintain accurate client records and HIPAA-compliant documentation. Coordinate laboratory testing and prior authorizations, while participating in team meetings and clinical supervision. Maintain the medication room and program areas, and provide training on activities of daily living. Shifts include day, evening, and weekend coverage with rotating overnight and on-call duties. Full-time role requiring 40 hours weekly, valid driver's license, and current RN or LPN licensure in Virginia.
Required Qualifications
- Registered Nurse (RN) or Licensed Professional Nurse (LPN) in the Commonwealth of Virginia
- Two years of nursing experience
- Experience working with mental health and/or substance use disorder clients
- Valid driver's license
- Acceptable driving record
- Mandt Training
- CPR Certification
- First Aid Certification
- AED Training
- HIPAA Access Level 2
- Ability to drive for long periods of time
- Must be able to walk distances, climb and descend stairs
- Must be able to lift/push/pull up to 25 pounds
- Regular and reliable attendance
- Thorough knowledge of the principles and practices of nursing
- Adherence to the code of ethics of the nursing profession
- Knowledge of principles, practices, and techniques involved in the diagnosis and treatment of individuals with mental health, and co-occurring disorders
- Knowledge of mental health, substance abuse and co-occurring, treatment interventions
- Basic knowledge about mental illness and the use of psychotropic medication in its treatment
- Ability to assess both general health problems and medication side effects
- Ability to organize and attend to detail in the delivery of medication
- Good interpersonal skills
- Ability to deal positively and constructively with changes and new challenges
- Demonstrated good judgment

- Provision of medication, health, and wellness education
- Training and support with activities of daily living
- Performance of crisis intervention
- Facilitation of prescreening/hospitalization
- Assistance with hospital diversion, hospital care coordination, and hospital discharge/re-integration services
- Documentation of client contacts and maintenance of client records
- Assistance with discharge planning and process
- Use of available technology including computers, e-mail, and voice mail
- Maintenance of effective relationships with co-workers and customers based on courtesy, compassion, and respect
- Participation in team meetings, clinical staffing and supervision
- Participation in professional development training, seminars, and conferences
- Rotation of day, evening and weekend shift coverage including overnight and weekend rotation of on-call coverage
- Availability for weekend shifts
