Job Summary
Conduct daily community visits for consumers with serious mental illness, performing initial and comprehensive assessments, wellness plans, and individual psychotherapy using evidence-based modalities. Maintain shared charts, progress notes, and service plan reviews per agency standards while participating in daily and weekly clinical meetings to incorporate findings. Act as an advocate and liaison for families, support systems, landlords, and community resources, providing psycho-education and relapse prevention strategies. Deliver 24-hour crisis intervention on-call services on a rotating basis. Requires a Master's degree in mental health counseling or social work with at least one year of experience working with individuals with serious mental illness and families.
