Job Summary
Conduct daily community visits to provide crisis intervention, stabilization, and individual psychotherapy using evidence-based modalities. Complete shared assigned charts, including initial assessments, service plans, and comprehensive reviews every six months. Participate in morning and weekly clinical meetings to discuss family issues and incorporate findings into service plans. Involve families and significant others in treatment through psycho-education and relapse prevention strategies. Act as an advocate and liaison for consumers with family, support systems, landlords, and community organizations. Provide 24-hour crisis intervention on-call services on a rotating basis. Requires a Master's degree and at least one year of experience working with individuals with serious mental illness.

- licensure in NYS
- one year of experience working in human services with individuals with serious mental illness
- experience in work with families
