ACT Forensic Registered Nurse

Coordinate the health care needs of assigned consumers by assessing psychiatric and physical health, developing individualized treatment plans, and implementing behaviorally specific interventions. Administer medications and injections, monitor status in acute settings, and negotiate health care responsibilities with provider agencies. Facilitate continuity of care by accompanying consumers to appointments, coordinating medical, vision, and dental services, and participating in team meetings to review medication and personal plans. Maintain comprehensive medical records, complete annual health assessments, and document all billable services according to CTT policy and National Standards. Provide consultation and educational training to consumers, families, and community agencies to enhance independence living skills.
