ACT Family Specialist

Job Summary
Establish family support systems and natural networks for clients to expand social and community interventions. Lead clinical meetings on family issues, integrate goals with team tasks, and provide individual and group psycho-education. Serve as advocate and liaison for clients regarding family, social, community supports, and landlords. Participate in developing comprehensive and six-month service plans, completing needs and follow-up assessments, and all required training. Deliver crisis intervention including rotating 24-hour on-call services. Complete progress notes, data recording, and monthly face-to-face treatment contacts. Address client needs for problem-solving, wellness self-management, housing, income support, education, vocational training, and primary care. Provide culturally competent services using necessary computer skills for reporting and documentation.
Required Qualifications
- Licensed professional
- Experience understanding family systems based on diversity of cultural and ethnic backgrounds
- Knowledge of ACT
- Knowledge of family support services
- Knowledge of advocacy groups
- Knowledge of community resources
- Computer skills necessary to meet online reporting and documentation requirements
- Computer skills necessary to meet referral requirements
- Computer skills necessary to meet research requirements
- Computer skills necessary to meet data reporting requirements
