ACT Case Manager

Job Summary
ACT Case Manager works in a multidisciplinary team to provide intensive case management services to adults with severe and persistent mental illness who are at risk of unsuccessful independent living. Provides services through a strengths-based approach, supports clients in adapting to life situations, developing independent living skills, and delivering educational resources including psychosocial groups. Responsibilities include attending ACT team meetings, delivering direct case management to adults, assisting clients and families to identify and secure community resources, coordinating referrals and services among community agencies, employment contacts, and the organization, designing individualized care plans based on a strength assessment, providing back-up to ACT specialty services, maintaining complete records per licensing/funding/CrossWinds standards, submitting daily progress notes and reports, building knowledge of community resources, maintaining professional relationships with clients and partners, attending professional workshops to maintain licensure/certification, and travel within Lyon & Chase Counties as needed.
Required Qualifications
- Associate’s degree plus two years’ experience in health and human services field OR four years’ experience in health and human services field is required
- Bachelor’s degree in psychology, social work, or counseling is preferred
- One year of experience working with SPMI population is required
- Must complete Case Management Training and provide certificate within 6 months of hire
- Complete training in one or more specialized services
- Computer experience in Microsoft Office programs and basic computer fluency
