Job Summary
Conduct intensive case management and problem-solving interventions for chronically mentally ill clients within the Assertive Community Treatment Team. Initiate assessment, treatment planning, and scheduling while providing direct clinical care through individual counseling, psychoeducation, and advocacy for independent community living. Participate in daily team meetings, interdisciplinary service planning, and 24-hour rotating on-call crisis coverage. Educate clients and families regarding mental health symptoms, medication effects, and compliance. Represent the program in the community to support persons with mental illness and their families. Engage in professional development to enhance intervention strategies and maintain state licensure.
Required Qualifications
- Bachelor's degree in Social Work from an accredited university
- Bachelor's degree in Nursing
- Licensure with the State of Michigan
- Ability to be credentialed by NLCMHA to provide services in this professional discipline
- Must maintain licensure
- Must be able to lift 25 lbs.
- Must be able to stretch/reach
- Must be able to stoop (bend at waist)
- Must have hand/finger dexterity
- Must have hearing in person and on the telephone
- Must have vision for close work

- Licensed Bachelors Social Worker (LBSW)
- Limited Licensed Bachelors Social Worker (LLBSW)
- Licensed RN
