Job Summary
Conduct intensive case management and problem-solving interventions for chronically mentally ill clients within an Assertive Community Treatment team. Initiate ongoing assessment, treatment planning, and scheduling while assessing emotional well-being, clinical symptoms, and medication effects. Provide direct clinical care through individual and family counseling, psychoeducation, and advocacy for independent community living. Participate in 24-hour rotating on-call coverage for crisis intervention and actively engage in interdisciplinary team meetings and staff development sessions. Represent the program in the community to educate families and assist in developing inter-agency relationships.
Required Qualifications
- Bachelor's degree in Social Work from an accredited university
- Bachelor's degree in Nursing
- Must have a licensure with the State of Michigan
- Ability to be credentialed by NLCMHA to provide services in this professional discipline
- Must maintain licensure
- Must be able to travel by personal vehicle if an agency vehicle is not available
