Job Summary
Nurse Care Manager position requiring an LPN with care-management background to lead comprehensive assessments, care planning, medication reconciliation, and care coordination for MassHealth ACO members. Work within an interdisciplinary team to enhance member health, support self-management, and navigate medical, behavioral health, and social services. Responsibilities include member education, facilitating referrals, potential home visits or in-person engagements, and accurate EHR documentation; travel within the assigned area may be necessary. On-site in Springfield, MA with a 10,000-sign-on bonus.
Required Qualifications
- Licensed Practical Nurse (LPN) with Care Management experience
- Associate degree in Nursing (ASN) or Bachelor’s degree in Nursing, BSN (preferred)
- Current, active MA Nursing license
- Minimum 2-5 years of nursing experience in community public health, case management, coordinating care across multiple settings, and with multiple providers
- Case Management Certification (CCM, ANCC RN-BC) preferred
- A valid driver's license and provision of a working vehicle
