Job Summary
Coordinate treatment plans with supervision, delivering interventions and implementing community activities that foster valued roles and relationships. Provide side-by-side support and coaching for 1:1 and group sessions to increase readiness for rehabilitation. Act as a liaison between persons served, families, and community agencies to secure employment, education, and entitlements. Participate in the on-call rotation. This role supports individuals with mental illness or dual diagnosis transitioning to independent living within a multidisciplinary team at Open Sky Community Services in Worcester.
Required Qualifications
- High School Diploma, GED or equivalent
