Job Summary
Conduct community outreach for adults with chronic behavioral health issues, including substance use disorder and schizophrenia. Advocate for clients by recommending resources and ensuring access to needed services. Perform assessments, address client needs, and make treatment recommendations while serving as the primary liaison with families and guardians. Provide documentation by updating electronic health records at the Braintree office site. May provide transportation, administer medication, and manage client finances. Maintain a large case load within a multi-disciplinary team. Requires a valid driver's license, CPR/First Aid, and MAP certifications.
Required Qualifications
- Current valid driver's license in good standing
- Driving experience
- Excellent driving record
- CPR certification
- First Aid certification
- MAP certification
Desired Qualifications
- HS Diploma
- BA degree in Psychology or related field
- One or more years of experience in the Mental Health field
- Working knowledge of community based treatment
- Working knowledge of funding sources
- Working knowledge of treatment regulations
