Job Summary
Develop and expand relationships with referral sources including hospitals, CHHAs, MLTC plans, and community partners to drive growth in the Personal Care Services Division. Serve as the primary point of contact for assigned accounts, ensuring consistent communication, responsiveness, and service excellence through regular in-person and virtual visits. Proactively identify new business opportunities, track referral activity and conversion metrics, and collaborate with Intake and Recruitment teams to ensure timely case conversion and service initiation. Resolve service issues raised by referral sources or patients while maintaining accurate documentation of all account interactions. Meet and exceed monthly and quarterly revenue targets in this commission-driven role.
Required Qualifications
- LPN (Licensed Practical Nurse), LMSW (Licensed Master Social Worker) or other graduate degree
- Ability to handle multiple tasks
- Ability to problem solve
- Computer literate in fiscal management and other database programs
- Demonstrate trustworthiness and discrete disposition in carrying out regulatory tasks
- Excellent written and oral communication skills
- Possess excellent interpersonal skills and ability to work well with others
- Proficient in Microsoft Office and CRM/database systems
Desired Qualifications
- Prior experience in home care, healthcare marketing, intake, or account management
- Proven ability to build and maintain professional relationships within the healthcare community
- Demonstrated success in a commission-driven or performance-based role
- Understanding of Personal Care Services (PCS), MLTC plans, and home care operations
- Highly motivated, results-driven, and comfortable working in a fast-paced, target-oriented environment
- Strong organizational and time management skills with the ability to manage multiple accounts
