Job Summary
Account Manager responsible for expanding membership through strategic community outreach, referral source development, and relationship management with physician offices, home care agencies, assisted living facilities, and community organizations. Conduct daily outreach visits to strengthen relationships with new and existing referral partners, educate them on the Plan’s benefits, and drive member referrals. Promote the Plan within the community through marketing activities, presentations, and ongoing engagement to increase awareness and membership growth. Maintain documentation of outreach activity, follow-up, meet assigned quotas, and participate in team meetings to support growth targets.
Required Qualifications
- Prior experience in Home Health Care preferred.
- Knowledge of Medicaid/Medicare regulations and managed care plans required.
- Proven ability to build and maintain referral source relationships.
- Ability to work independently while collaborating with leadership.
