Job Summary
Access Liaison responsible for connecting consumers with the health system, handling acquisition, front door navigation, care coordination and transitions of care. Manages incoming contacts via phone, live chat, email, and text to determine the correct pathway and next steps, coordinates with clinic partners to remove access barriers, and builds relationships with internal/external stakeholders. Creates patient-focused options by addressing preventive care screenings and annual appointments, delivering first-contact resolution, and ensuring timely, effective care outcomes while maintaining a patient-centered approach across multiple systems.
Required Qualifications
- High school diploma or equivalent (Education Required)
- Four years of experience in customer service, appointment coordination or related field (Work Experience Required)
- Proficient in navigating multiple systems
- Excellent interpersonal and stakeholder management skills
