Job Summary
The Access Case Manager evaluates medical necessity for patient admissions and care unit placements, coordinating discharge planning and referrals for patients with complex needs. The role requires engaging with payers for reimbursement, working alongside medical teams, and addressing patients' financial or psychosocial needs. Key qualifications include a Bachelor's Degree in Nursing or related field, recent hospital experience (preferably ICU, CCU, or 3 years Med/Surg), and knowledge of relevant healthcare policies.
Required Qualifications
- Recent hospital experience preferably ICU, CCU, or strong minimum 3 years Med/Surg experience
- Knowledge of Pathophysiology and Disease Process
- Knowledge of Medicare, Medicaid, Managed Care Health Plans
- Current American Heart Association BLS required
Desired Qualifications
- Certification in Case Management preferred or obtained certification in Case Management within two years of hire
- Experience in Hospital Case Management preferred
