Implement selected components of care and interventions to improve patient functional independence in an outpatient home setting. Provide skilled therapy for geriatric clientele in Assisted Living Facilities while communicating with families, physicians, and the broader health care team. Maintain timely, thorough documentation of all services in the medical record. This role requires an actively licensed Physical Therapist Assistant with an accredited Associate's degree and two years of clinical experience. Compensation is $70 per treatment session, with flexible scheduling and the ability to accept or decline patient assignments.
